Melanie Tan is a population health executive who has led growth, transformation, and value based care strategy across providers and mission driven health organizations. She is recognized for launching and scaling innovative VBC models, driving multi state expansion, and delivering measurable improvements in clinical, financial, and operational performance.
Most recently, Melanie served as Vice President of Strategy & Growth at Medical Home Network, where she led enterprise wide strategy, payer contracting, and new market growth for safety net–focused value based care initiatives. She oversaw the launch of seven Medicare ACO REACH and MSSP ACOs—achieving a 100% CMS award success rate, expanding covered lives 5X over four years. She spearheaded organizational repositioning efforts, including MHN’s transition to a Public Benefit Corporation enabling mission expansion, and served as a key executive liaison to CMS/CMMI and state agencies.
Melanie is the Founder and Principal Consultant at Valunova Health Consulting, advising ACOs, CINs, provider organizations, FQHCs and population health companies on VBC growth strategy, ACO performance improvement, CMS model evaluation, managed care contracting, and go to market planning. She has developed accelerator frameworks that help organizations rapidly operationalize value based programs and improve readiness for VBC models, including ACO REACH and MSSP.
Earlier in her career, Melanie spent nearly a decade at Deloitte Consulting, where she led more than 20 enterprise performance improvement initiatives for health systems and physician enterprises nationwide. Her work generated financial impact, spanning clinical productivity redesign, cost and quality optimization, revenue cycle transformation, and care model innovation. She also contributed to international health system reforms in China and Australia.
In addition to her advisory work, Melanie is an Adjunct Professor at the University of Illinois Chicago School of Public Health, where she teaches Population Health management to graduate and executive students, bringing real world ACO and VBC implementation experience into the classroom.
Melanie holds an MHSA and BS from the University of Michigan School of Public Health and is a certified Project Management Professional (PMP). She serves as Board Member & Chair of the Quality Improvement Committee at Esperanza Health Centers and is an active mentor to emerging public health leaders.
